The first week on a GLP-1 medication is typically underwhelming by design. Here is what is actually happening physiologically and what to expect day by day.

What Appetite Suppression Actually Feels Like

Patients consistently describe GLP-1 appetite suppression differently than they expected:

What it is not:

• Nausea forcing you not to eat (at therapeutic doses in most patients)
• Dramatic loss of interest in all food
• No hunger at all

What it typically is:

• Food feels less mentally compelling — the "food noise" quiets
• Hunger arrives but feels less urgent, easier to delay
• Fullness arrives sooner into a meal and feels more comfortable to respect
• Portions naturally shrink without deprivation
• Previously habitual eating (snacking out of boredom) becomes less automatic

For many patients, the first week reveals early versions of this — a meal that previously seemed small now seems adequate, or a snack they'd normally want no longer appeals.

Managing Week 1 Side Effects

If you have nausea:

• Eat small amounts (crackers, toast, plain rice) rather than full meals
• Stay upright for 30–60 minutes after eating
• Avoid high-fat, fried, or spicy foods entirely this week
• Try ginger tea or ginger chews
• Take your injection in the evening if morning nausea is problematic

If you have digestive changes:

• Diarrhea or loose stools: stay hydrated; usually self-resolving in days

• Bloating: avoid carbonated drinks; eat slowly; small portions

If you feel nothing:

• This is normal and fine. Do not increase your dose. The titration schedule exists for good reason.

1. Let pen reach room temperature (~30 minutes out of fridge)
2. Clean injection site with alcohol pad; let dry completely
3. Pinch skin gently at the injection site
4. Press pen firmly against skin at 90-degree angle; press button
5. Hold for 10 seconds before removing
6. Do not rub the injection site

1. Your provider will supply a dosing card — follow it exactly for units
2. Use the provided insulin syringe; draw to the prescribed unit line
3. Inject subcutaneously at abdomen, outer thigh, or upper arm
