The starting dose reality: 0.25mg semaglutide is pharmacologically active but well below the therapeutic range (1.7–2.4mg). Think of week 1 as your body learning to tolerate the medication class, not as the medication working at full effect.


What Appetite Suppression Actually Feels Like

Patients consistently describe GLP-1 appetite suppression differently than they expected:

For many patients, the first week reveals early versions of this — a meal that previously seemed small now seems adequate, or a snack they'd normally want no longer appeals.


Managing Week 1 Side Effects

If you have nausea:

  • Eat small amounts (crackers, toast, plain rice) rather than full meals
  • Stay upright for 30–60 minutes after eating
  • Avoid high-fat, fried, or spicy foods entirely this week
  • Try ginger tea or ginger chews
  • Take your injection in the evening if morning nausea is problematic

If you have digestive changes:

  • Diarrhea or loose stools: stay hydrated; usually self-resolving in days

  • Bloating: avoid carbonated drinks; eat slowly; small portions

If you feel nothing:

  • This is normal and fine. Do not increase your dose. The titration schedule exists for good reason.
